Admissions Requirements: Admissions

Freshmen applications for public school graduates

FORM 137 (Permanent School Record)

A copy of the scanned Form 137 must be submitted on the link provided in Admissions Form A for your application evaluation.

ADMISSIONS FORM A

Upon reaching Step 2 of the application process, you are required to encode your personal information and academic records to verify your identity and qualification for admission.

2x2 PICTURE

This must be attached to your Step 1 e-mail in order to allow the Admissions Office to verify your identity before the Office issues you your application number.

WORKING GOOGLE ACCOUNT

This Google Account must be properly working in order for you to access the Application Form. If you elect to use an existing e-mail, check if your account has been compromised (refer to https://haveibeenpwned.com/) to ensure that you will not lose access to your account. The Admissions Office will only accept the first e-mail that you have used for Step 1.

ADMISSION FEE (Php 100)

Students from private schools must pay the small application fee. For students who took advantage of the voucher program, request a proof from your school in order to waive this fee.

Freshmen applications for foreign students

TRANSCRIPT OF RECORDS

A copy of the student's Transcript of Records must be uploaded on the link provided in Admissions Form C for application evaluation.

TOEFL PASSER

A TOEFL score of at least 550 points for the paper-based test or a score of at least 84 for the Internet-based test is required for campus admission. This is a requirement for non-native English speakers; applicants from North America or Great Britain are not required to take the TOEFL.
